中山醫學大學附設醫院今天舉辦「中山大雁、共榮基層」啟航儀式，與19家地區醫院、250家診所、3家居家護理所合作，推動分級診療及雙向轉診。

黃建寧表示，醫院和基層診所透過雙向轉診系統，主動把病人轉到基層診所，除牽涉到醫院和基層診所之間的互信，還包括病人是否信任這個轉系，中山會盡力做好。
李伯璋指出，落實分級醫療，絕對不是要病人轉診就好，醫學中心要當基層診所的後盾，彼此互相信任，提供民眾更優質的醫療照護，大家有默契，一起努力。
中山醫學大學附設醫院與南投醫院等19家地區醫院、250家診所及3家居家護理所簽訂策略聯盟，將雙向轉診快捷化，每天保留600個門診名額給轉診病患。
轉診中心提供掛號服務單一服務窗口，由專人辦理轉介病患，建立轉診民眾綠色通道，並提供轉診優惠，給予轉診病患完善便捷的醫療照護。
這個體系推動「雙主治醫師」制度，當病患轉診到醫院時，即時發送簡訊通知診所，以利原轉介診所醫師掌握病人就醫動態，藉由「智慧轉診平台」與醫院主治醫師雙向溝通，有效連結轉診病患回到社區診所的後續醫療照護。
「中山大雁、共榮基層」策略聯盟將落實分級診療，使各層級醫院診所能發揮所長、充分利用資源，透過照護聯盟計畫執行，主動轉介符合計畫個案至合適的診所及地區醫院。
